- July 31, 2025: TB and Immunization Requirements due
- August 1, 2025: First day of SHIP and UHS eligibility at student pricing; First day to schedule UHS appointments for missing vaccines/titers
- August 27, 2025 (First Day of Instruction): COVID-19 Requirement Due
- October 1, 2025: Enrollment holds for non-compliance for TB and Immunizations (NOT including COVID-19) posted to Cal Central (impacts Spring 2025 enrollment)
- November 1, 2025: Annual Flu and COVID-19 vaccine requirements due. To comply with the Flu and COVID-19 Vaccine Policy by November 1, 2025, all students need to either get the flu and COVID-19 vaccine and log those records into eTang Medical Clearance section or submit a declination for the vaccine on eTang. Submit one for each vaccine you are opting out of.
- Please note: For Fall 2025, students should continue to use eTang to meet the requirements, complete forms and submit documentation in Medical Clearances. You can also still send a secure message to the Mandatory TB & Immunization team through eTang messaging.

- Mandatory TB and Immunization MyChart Instructions for completing requirements in MyUCBerkeleyHealth.
- Action you can do now is to activate your MyChart account in MyUCBerkeleyHealth. See MyChart Activation Instructions.
- December 31, 2025: TB and Immunization Requirements due
- December 31, 2025: COVID-19 and Influenza (Flu) Vaccine Policy Requirement due
- Enter a record of the COVID-19 and/or Flu vaccine or submit a declination request in MyUCBerkeleyHealth.
- January 1, 2026: First day of SHIP and UHS eligibility at student pricing; First day to schedule appointments for missing vaccines/titers
- March 1, 2026: Enrollment holds for non-compliance for TB and Mandatory Immunizations posted to Cal Central (impacts Summer and Fall 2025 enrollment). Concurrent enrollment students will not get an enrollment hold for non-compliance with TB and immunization requirements. But we strongly recommend that you complete all these requirements before coming to campus.
- Note: For Spring 2026 requirements, there are NO late fees since we are migrating to a new electrontic health records system.
Note: MFE students will be in Summer 2026 incoming and should follow those deadlines.
Notice For Spring 2026 students: all requirements must be completed through the MyUCBerkeleyHealth portal.

- If you haven't already activated your MyChart account, students were sent an email on Monday, Nov. 17th to your berkeley.edu address with a MyChart Activation Code (this is the easier way to active your account)
- Tip: If you are trying to activate your account without a code and you enter your address and it doesn't work, try again, using our UHS address and zip code: 2222 Bancroft, Berkeley CA 94720.

Mandatory Immunization Requirements

Please note the following:
- All incoming students must also complete a Tuberculosis risk questionnaire and may have to submit TB test results and/ or additional medical clearance documentation.
- TB testing can now be done without regard to the timing of COVID-19 vaccinations. The timing for TB testing after a live virus vaccine (MMR and Varicella) still needs to be one month.
- There is a season/annual Influenza (flu) and COVID-19 vaccine requirement. Information about these vaccine requirements, including declination process and due dates comes out every Fall.
- Students who falsify any immunization or TB screening information that is submitted will not be compliant with the requirements and may face disciplinary action if found to have violated the Berkeley Campus Code of Student Conduct (see Section V, Grounds for Discipline).
