TB and Immunization Fairs

For Fall 2024 Incoming and re-admitted students 

  • August 19-23 and August 26-30
  • By Appointment booked on eTang
    • Fair appointments will be available for booking by August 5th at 9 am. 
    • To book an appointment go to eTang > appointments > TB & Immunizations > Mandatory Immunizations

Location

TB and Immunization Fairs are held at University Health Services Tang Center, 2222 Bancroft Way, Berkeley. 

Important dates

  • The formal due date for completing TB/Immunization Requirements is July 31, 2024  (August 28 for COVID-19)
  • Late fees will be charged for students who are not compliant by September 1, 2024
  • Enrollment holds will be placed for students who are not compliant with TB Clearance, and Immunization requirements by October 1, 2024

Before the Fair

Make sure to complete these items on eTang before the fairs:

  1. Upload your immunization records to the eTang ‘Medical Clearances’ page
  2. Enter the dates of the required immunizations on the Medical Clearances page
  3. Complete the following forms on eTang:
    1. TB Screening Questions (on the Medical Clearances page)
    2. Notice of Privacy Practices (on the Medical Clearances page)
    3. Statement of Financial Responsibility (on the Medical Clearances page)
    4. If you will be under 18 years of age by the dates of the fairs, submit the Minor Consent Form (go to ‘Messages’, select ‘New Message’, and choose the option to submit the Minor Consent/Compliance Form to the Health Records department, and follow the prompts).
  4. Read the Vaccine Information Statements for vaccines you will need
    1. Tdap (Pertussis)
    2. Measles, Mumps, Rubella
    3. Varicella (Chickenpox)
    4. Meningococcal ACWY
    5. Pfizer-BioNTech COVID-19 Vaccine

What to do on the day of the Fair

  • Eat a hearty breakfast or snack and drink water in preparation for blood tests or immunizations.
  • Be prepared to stay for 15-30 minutes in the waiting area after your vaccine.
  • It is best to wear a short-sleeved shirt or dress in layers so that it is easier to collect blood tests or administer vaccines.
  • Bring your Cal1Card OR a photo ID and your Student ID number.
