Please complete this sample assessment to apply to the Eat Well Berkeley Restaurant Program. The process can be fairly easy, and may involve adding a few healthy choices, adjusting your beverage product mix, and/or using simple strategies to promote the healthier choice. Email your completed assessment to Kim Guess, RD at firstname.lastname@example.org.
You will receive feedback on your assessment and if improvements are needed, you can make changes and submit the assessment again. You may choose to use our Eat Well Berkeley icon to label healthy items that meet our criteria on your menu (must be approved by our dietitian). You may also choose to have a nutrition consultation.
Once the assessment reaches a satisfactory level, you will receive promotional materials to display in your restaurant or retail market and we will list your business on our webpage!
If you would like to use Eat Well Berkeley labelling on the healthy items on your menu, our dietitian will work with you to determine which items meet our standards. Menu items may only be marked with our check mark once they are approved by our dietitian. If you add new items to your menu or have a new seasonal menu, you must get approval from our dietitian before adding the check mark label.
As students, staff, faculty, and visitors, we can also help make our campus environment healthier.

Why Become an Eat Well Berkeley Partner?
If you are a university-run restaurant or retail market, or a third-party lease on campus, you are required to comply with the Food & Beverage Choices Policy. Once you are in compliance, you will automatically become an Eat Well Berkeley partner and receive all the benefits!
If you are not required to comply with the policy (independent campus restaurants without a lease, off campus restaurants, etc.), you can voluntarily participate. We use the same assessment as the one we use for restaurants included in the policy.
