Waiving SHIP

Fall 2018 Waiver Information 

The Fall 2018 waiver period is May 1, 2018 to July 31, 2018. Waivers must be received by July 15, 2018 to avoid a late fee of $75. All waivers received between July 16, 2018 and July 31, 2018 will be subject to a late fee of $75.
The  2018-19 waiver criteria, waiver worksheet and other resources are provided below under Waiver Resources.

ASUC Student Advocate's Office

The ASUC Student Advocate's Office is a student-run office that provides free and confidential advice and representation to students who experience issues or conflicts with the university.   1. This waiver application is only for students in a Summer Special Program who are waiving for the Summer 2018 session.

International Summer Session Students

Welcome to Berkeley!

All international students attending a UC Berkeley Summer Session must have health insurance that includes medical benefits and medical evacuation and repatriation of remains coverage in compliance with their visa requirements. Please contact or visit the Berkeley International Office for assistance with requirements and other resources. 

Special Summer Programs

Berkeley SHIP information for Special Summer Programs 2018

The information on this page is only for the following programs: Professional Track LLM, LLM Hybrid Option (summer starts only) and incoming students in the following programs: Master of Public Affairs, Masters of Arts in Teaching, Master of Real Estate Development & Design, and Master of Financial Engineering (incoming March class only).

Prescriptions

Please note: As of August 1, 2018 - Pharmacy co-pays will be changing. Please check back for more updated information in the next few weeks.

At University Health Services Pharmacy

Spring 2018 promo: Prescriptions filled at the UHS Pharmacy have a $0 co-pay for generic, $20 co-pay for brand name medications, and $35 co-pay for "non-formulary" items (30-day supply). Birth control is covered at 100%; some conditions apply.

Medical - Outpatient

1A referral by a UHS Clinician is required BEFORE seeking services outside of UHS or your claims will be denied.

2Subject to the $300 deductible. 

3If the student's plan terminates before the 31 days from date of birth, the newborn's coverage ends on the same termination date as that of the student.
