To save you time and frustration, please take a moment to review these helpful steps. Most errors encountered during the waiver process can be avoided by following these simple instructions. 

  1. This waiver application is only for students in a Summer Special Program who are waiving for the Summer 2018 session.
  2. Please find your program below for the Summer 2018 waiver period.

Waiving SHIP

Spring 2019 Waiver Information

The Spring 2019 waiver period is December 1, 2018 to January 1, 2019.  The late waiver period is January 2, 2019 to January 15, 2019.  
All waivers received between January 2, 2019 and January 15, 2019 will be subject to a late fee of $75.

SHIP Referrals

Don't get stuck with the bill!

An authorized referral is required prior to seeking medical services outside of Tang or your claims will be denied. Dental, vision, services provided in a hospital emergency room or urgent care clinic, prescriptions filled outside of UHS, international services, and some limited preventative and women's health visits do not require an authorized referral.
