SHIP Insurance now covers at-home COVID testing kits

Starting on January 15, 2022, SHIP Insurance will pay for eight at-home COVID-19 tests per person, per month. 

The SHIP office is coordinating with Wellfleet to address both immediate plans we can readily execute and others that will take a little time to bring together. 

Currently, guidelines have been established for the following:

Coronavirus Testing (OTC) without a doctor order

Effective for tests purchased on or after January 15, 2022, SHIP/Wellfleet will cover over-the-counter (OTC) COVID-19 tests without a doctor order limited to 8 tests per calendar month per covered member with no member cost share. OTC tests with a doctor order have no limit. The test must be purchased for personal use, not for employment purposes, has not been (and will not be) reimbursed by another source, and is not for resale. The following documentation must be submitted:

  • COVID-19 OTC Test Reimbursement Form - must be completed and signed.
  • Receipt from the seller, documenting the date of purchase and price
  • Actual UPC label from the COVID-19 test box

Reimbursement Process

Any FDA-approved COVID-19 take-home test kits purchased at local pharmacies and retailers after January 15, 2022, are eligible for insurance reimbursement. The reimbursement form must be filled out after paying for a testing kit. Currently, under this process, the OTC Test Reimbursements will be handled as Medical Claims, and HealthComp will process these, with no cost to the member or impact to the plan for the allowed eight tests per month for non-physician-ordered scenarios.

To be eligible for reimbursement the following must apply:

  • Must have been purchased on or after January 15, 2022
  • Must be approved by the Food and Drug Administration
  • Must be purchased for personal use
  • Must not be for employment purposes
  • Has not been (and will not be) reimbursed by another source
  • Not for resale

Please include the following documentation with your submission:

  • Completed and signed COVID-19 OTC Test Reimbursement Form
  • Receipt from the seller, documenting the date of purchase and price
  • Original UPC label from the COVID-19 test box

Upon completion, attach the required documents and send this form to:

Keep copies of your original documents for your files. We cannot return originals to you.

A claim can also be submitted online after you have created your Wellfleet account. The steps are as follows:

  1. Go to berkeley.wellfleetinsurance.com/ship and create or log into your account.
  2. Click on "Forms" on the Homepage.
  3. On the left side, select "COVID Test"
  4. Complete the form and submit the requested documentation:
    • Receipt from the seller, documenting the date of purchase and price
    • Origional UPC label from the COVID-19 test box

If you don't have SHIP

All private health insurance plans—both group health plans provided through employers and those purchased in the individual marketplace—are required to pay for eight at-home COVID-19 tests per person, per month. Contact your health insurance to see how they will be processing reimbursements for at-home COVID tests.
