SHIP for Non-Registered Students
Who May Enroll?
A voluntary SHIP option is also available to those who have approved temporary medical withdrawal or pregnancy leave from the University while retaining student status. The student must have been enrolled in SHIP the Fall or Spring term immediately preceding the term for which the student wants to purchase. The Berkeley SHIP office will evaluate eligibility on a case-by-case basis.

Effective Fall 2018, SHIP is not available to students who have graduated or are on non-medical/pregnancy leave from the University. For help transitioning off of SHIP, please see our tips here.
NOTE: Filing Fee, Concurrent Enrollment, and medical withdrawal students are NOT automatically enrolled. If you purchase the voluntary plan, you are also eligible to add your dependents under the voluntary plan. Please see the information available on Insurance for Dependents for details.

How to Enroll?

Step 1: Fill out enrollment form:
Fill out all fields on the front and back of the enrollment form as needed. Check the appropriate box(es) on the rate charge to select the term of coverage.
Step 2: Obtain approval:
Bring the enrollment form to the Berkeley SHIP office (2222 Bancroft Way #3200, Berkeley, CA 94720) for approval.
Step 3: Send form to USI:
After approval, send the enrollment form, along with payment, by mail, email, or fax to:

*Enrollment in the voluntary plan for those on temporary medical withdrawal or pregnancy leave may be accepted on a rolling basis within the semester and eligiblity will be determined on a case-by-case basis. Please note, there is no pro-ration for late enrollments.
