SHIP for Non-Registered Students

Who May Enroll?

Filing fee graduate or concurrent enrollment undergraduate students may purchase the voluntary plan for a maximum of one semester. The student must have been enrolled in SHIP the Fall or Spring term immediately preceding the term for which the student wants to purchase. 

A voluntary SHIP option is also available to those who have approved temporary medical withdrawal or pregnancy leave from the University while retaining student status. The student must have been enrolled in SHIP the Fall or Spring term immediately preceding the term for which the student wants to purchase. The Berkeley SHIP office will evaluate eligibility on a case-by-case basis.

Effective Fall 2018, SHIP is not available to students who have graduated or are on non-medical/pregnancy leave from the University. For help transitioning off of SHIP, please see our tips here.

NOTE: Filing Fee, Concurrent Enrollment, and medical withdrawal students are NOT automatically enrolled. If you purchase the voluntary plan, you are also eligible to add your dependents under the voluntary plan. Please see the information available on Insurance for Dependents for details.

Filing Fee (Graduate students only)

Graduate students must apply to their academic departments for official Filing Fee status. Filing Fee status is in lieu of registration to either file a dissertation, file a thesis or to take a comprehensive exam. Filing Fee students are not allowed to take courses while on Filing Fee.  Filing Fee can only be used once. To complete the Voluntary SHIP enrollment process, the student must submit a letter from his or her academic department confirming the student’s approved Filing Fee Status to the Student Health Insurance Office.

Concurrent Enrollment Status (Undergraduate students only)

Undergraduate students in their final semester of study who are completing degree requirements or recovering from academic probation through UC Berkeley Extension's Concurrent Enrollment Program are considered to be on Concurrent Enrollment Status. To complete the Voluntary SHIP enrollment process while on Concurrent Enrollment Status, a student must submit a letter of approval from his or her academic department and proof of payment for the class(es) or class schedule at UC Berkeley Extension to the Student Health Insurance Office.

Medical Withdrawal or Pregnancy Leave Status 

Graduate and undergraduate students who have a formal medical withdrawal status OR are on approved pregnancy leave AND were enrolled in the Plan the in the preceding semester as a registered student may be eligible for voluntary SHIP. Enrollment must be initiated by the student and is not automatic. You must pay the applicable insurance premium.  Contact the Berkeley SHIP office for more information including effective dates and for eligibility determination.

Undergraduates  $1,497 (one semester only)
Graduates  $2,373 (one semester only)

Enrollment Maximum

The SHIP voluntary plan is available for only one semester. 

How to Enroll?

Step 1: Fill out enrollment form: 
Fill out all fields on the front and back of the enrollment form as needed. Check the appropriate box(es) on the rate charge to select the term of coverage.

Step 2: Obtain approval:
Bring the enrollment form to the Berkeley SHIP office (2222 Bancroft Way #3200, Berkeley, CA 94720) for approval.

Step 3: Send form to USI:
After approval, send the enrollment form, along with payment, by mail, email, or fax to:

*Enrollment in the voluntary plan for those on temporary medical withdrawal or pregnancy leave may be accepted on a rolling basis within the semester and eligiblity will be determined on a case-by-case basis. Please note, there is no pro-ration for late enrollments.
