Transition/ Continuity of Care Resources

This process allows continuity of care for current SHIP members whose treating physicians are not part of the Anthem Blue Cross network. Students who are in the middle of certain treatment plans will be able to continue their care and recieve in-network benefits while they transition to the new health plan. Transition/ Continuity of Care Form

Transition/ Continuity of Behavioral Health Care

If care is currently being provided to a member by a provider not in Anthem's network, the member or the provider can request coverage for continuity of care. For behavioral health, standard transition/ continuity of care is authorized for 90 days with current provider, thus allowing time for the member to find a participating provider with or without Anthem's assistance, or to decide they want to remain with their current provider and pay the out-of-network costs. Transition/ Continuity of Pregnancy Care

If care is currently being provided to a member by a provider not in Anthem's network, the member or the provider can request coverage for continuity of care. Continuity of care for pregnancy is offered as long as the member was seen for their pregnancy prior to change in the insurance. Completion of covered services shall be provided for the duration of the pregnancy, regardless of trimester, and during the immediate postpartum period. Other Cases for Transition/ Continuity of Care

If care is currently being provided to a member by a provider not in Anthem's network, the member or the provider can request coverage for continuity of care. Each case is reviewed and discussed with the members current provider to see how much longer the services are needed and what types of services are needed. Based upon this and additional information a transitional plan is built.
