Waiver Information

UC Berkeley and Mills College students may opt out of SHIP coverage for the academic year 2008-09 by submitting a waiver application. If you do not submit a waiver you will be enrolled in SHIP. Students may also submit a waiver application for Spring 2009, between December 1, 2008 and January 2, 2009 only.

The deadline for submitting a SHIP waiver for Fall 2008 is July 15th, 2008. All waivers submitted after July 15th will be subject to a $50 late waiver service fee, billed to the students' E-Bill. No waivers will be accepted after September 15th, 2008.

Please note that students who have waived SHIP enrollment for Fall 2008 will have enrollment waived automatically for Spring 2009.

UCB undergraduate students: Waive SHIP for Fall 2008.

UCB graduate students: Waive SHIP for Fall 2008.

Mills students: Waive SHIP for Fall 2008.
