Spring 2016 WAIVER INFORMATION
The Spring 2016 waiver period is December 1, 2015 - January 10, 2016. Your waiver must be received by January 10, 2016 to avoid a late waiver fee of $50. All waivers received between January 11 and February 5 will be subject to a late fee of $50. No waivers will be accepted after February 5.
SHIP TERM COVERAGE PERIODS FALL TERM: August 15-January 14; SPRING TERM: January 15-August 14.

If you have submitted your online waiver and it was denied, you may submit a waiver appeal if you believe your insurance does meet the criteria. Your appeal must be submitted within 10 days in order for your waiver to be considered. You must have already completed the online waiver and received a denial to use this form.
To securely submit your SHIP Waiver Appeal, log in to the eTang patient portal with your Calnet ID and passphrase.
From the welcome page, click "MESSAGES" on the left side menu. Click "New Message". Under "More Options", select "SHIP waiver appeal". Fill out the form completely and click the "Send" button. If you do not receive a response to your appeal within 30 days, please contact the insurance office at (510)642-5700 or firstname.lastname@example.org. Please note, you may be contacted via secure message in the eTang patient portal if additional documentation is needed to process your appeal.
If you have waived SHIP enrollment for Spring 2016 and wish to reverse your waiver, you can fill out a Waiver Reversal form (PDF). If you do not have dual insurance (SHIP plus another health insurance plan) and SHIP will be your only insurance plan, you must show proof of termination of your former insurance plan.
