Waiver Information
FALL 2013 WAIVER INFORMATION
Waivers must be submitted each academic year. Students who wish to waive SHIP for Fall 2013 may begin the waiver process here. Your waiver must be received by July 15, 2013 to avoid a late waiver fee of $50. All waivers received between July 16 and September 15, 2013 will be subject to a late fee of $50. Absolutely no waivers will be accepted after September 15, 2013.

WAIVER APPEAL FORM
If you have submitted your
online waiver and it was denied due to inaccurate or incomplete
information, you may complete a Waiver Appeal form (PDF) and
resubmit as instructed. You must have already
completed the online waiver and received a denial to use this form. You
may also email the completed appeal form along with the required
documents to uhswaivers@uhs.berkeley.edu.
WAIVER REVERSAL
If you have waived SHIP enrollment for Fall 2013 and wish to reverse your waiver, you can fill out a Waiver Reversal form (PDF). If you do not have dual insurance (SHIP plus another health insurance plan) and SHIP will be your only insurance plan, you must show proof of termination of your former insurance plan.
