Important SHIP Changes for Plan Year 2009-2010

Temporary ID Cards available from Anthem Blue Cross

Effective August 15, 2009 for UC Berkeley:

• The Physical Therapy maximum will increase from $1,000 to $1,500 per plan year. SHIP will cover up to $1,500 of expenses for Physical Therapy for a student between August 15 and August 14 each plan year.
• Ground Ambulance services will be covered at 90% of customary and reasonable charges if the patient receives emergency treatment or is hospitalized. There is no longer a benefit limit and paid amounts will accumulate toward the out of pocket maximum of $3,200 per plan year.
• Services for Transgender Members: Hormone therapy is covered as any other prescription medication. Sex reassignment surgery is covered as any other surgery, up to a lifetime maximum of $75,000. Travel costs associated with surgery may be covered. See Plan Description booklet for complete information on benefits.
