About Headaches
Tension Headaches (pdf format)

Headache Diary (pdf format)
A form that helps you to keep track of your headaches, identify triggers,
and monitor changes in frequency and severity. It can help you and
your provider better understand your headaches. If you'd prefer an
interactive, on-line headache diary, try this tool available from
the American Council
for Headache Education.
Migraine Triggers (pdf format)
Triggers are specific factors that may increase your risk of having
a migraine attack. This fact sheet explores dietary, sleep, hormonal,
environmental, stress and physical triggers.
Migraine Disability Assessment Test
(MIDAS) (pdf format)
Your clinician may ask you to complete this form that helps to measure
the impact your headaches have on your life.
