Healthy Habits
Lifestyle Incentive Program

Healthy
Habits is a 12-week lifestyle incentive program to help you successfully
improve your eating and physical activity habits. Participants will
choose and track simple nutrition and physical activity habits
and will receive a weekly wellness bulletin. Click

Additionally, all registered participants will receive
a grocery tote and will be eligible for weekly prize drawings.
Registered participants who have been tracking their goals and provide
a copy of their log sheet to the program coordinator will be entered
into the Grand Prize Drawings at 6 and 12 weeks!
Prizes
include a Nintendo Wii, iPod Shuffle, memo boards, "I Heart Tap
Water" water bottles, exercise bands and more.

they type your name in as a referral on their registration form and
YOU'll get a bonus prize.
How do I register for the program?
Registration is closed for this program.
You will receive a Registration Confirmation email from
the Program Coordinator including electronic copies of your program
materials for you to print out. You may request hard copies of the materials
by clicking the appropriate button on the Registration Form or by contacting
the Program Coordinator at (510) 643-3434, pmielbeck@uhs.berkeley.edu.
You MUST register to be eligible for prizes.
How does the program work?
Every week you will choose and track a simple nutrition
habit and activity habit. The idea is to make small, simple
changes to your lifestyle to produce big changes in your health. Just
check the box for each day you complete your habit! It's that
easy.
Before you know it you'll be well on your way to feeling
great and full of energy, just in time for summer!
